Medical Minute: Diabetes Education

OhioHealth Grant Medical Center’s diabetes program is helping patients take control of their health through expert-led education, personalized care and ongoing support.

“Some patients are told over the phone that they have diabetes,” says Rachel Sayre, BSN, RN, CDCES Grant Diabetes Program Nurse Educator and Certified Diabetes Care and Education Specialist.

Getting told you have diabetes and to seek education over the phone can be daunting, which is where Sayre and her team come in to make it a little bit easier.

While new patient education is crucial, so is a touch-base every once in awhile. Diabetes affects every part of the body and understanding new technology and medications is the best way to avoid complications.

The program covers essential topics like understanding diabetes, its effects on the body, medication management, potential complications and any recommended lifestyle changes. Patients have the opportunity to meet one on one with a registered nurse educator and a dietitian to receive personal guidance. The program includes ten hours of diabetes education and a binder filled with resources from each session. Weekend sessions are even offered as an option.

To register for classes you need a referral from your primary care provider or endocrinologist. Don’t let this stop you from calling the office with questions about referral options or the program. They are always welcomed.

“We can help go about that process. We can help them place the order. We can help the patient learn more about the program and we can provide the options for the schedules,” says Sayre.
